Insurance

We accept all third party health insurance.
That includes Medicaid and Medicare.

Cash is also accepted.
Walk-ins welcome.

Insurance

The patient is expected to have the proper insurance information ready prior to the visit. This may include changing your PCP to the office PCP. Patients who fail to present this information will be asked to pay in full or make and confirm the change prior to being seeing or to reschedule the visit.

Deductibles, co-insurance and copays are due at the time of service.

The patient is expected to present an insurance card at each visit. All past due balances must be paid at the time of service.

Self-Pay Accounts

Self-pay are patients without insurance or patients who do not have their insurance card on file at the time of service. It is expected that payment is required at time of service for all services. 

Referrals

If your health insurance requires referrals be made, you are required to notify the referral department before you go to the appointment. In most cases, the patient will need to be seen in our clinic before any referrals are made. If you visit the ER or Urgent Care department over the weekend, you must contact the office the following Monday. Due to contractual obligations with your insurance company we will not be permitted to back date a referral date.
